110-300-0221(2)(a) Licensor observed no handwashing sink within arm’s reach of the diaper changing station or readily accessible to prevent cross contamination. 110-300-0221(2)(b) Licensor observed the diaper changing station not to be horizontally or vertically surrounded by washable material that extends at least two feet from the diaper changing station and handwashing area. 110-300-0221(4) Licensor did not observe a container designated for disposing of soiled diapers and diapering supplies that is hands-free, covered with a lid to prevent cross contamination, lined with a disposable plastic trash bag, and within arm’s length of the diaper changing area.
